1st & last name & a phone# or email address that you want me to use to contact you and a Provider Reference: The name & contact info of at least 1 reviewed provider that you have seen.

There are many risks in seeing new clients. I need this personal info only for verification purposes & will destroy it after our first meeting. If you are leary about giving this info to me, than I am not the girl for you. Remember you are coming to my private home, my safety is very important to me.
